The following forms will be needed for your lessons. If you right click on the form, you will see the option to "copy image". These forms were converted to jpeg files in order to input in the EBook. You will need to copy the image and paste in on a word document. Then, you will be to print the form for use in your lessons.

  1. Right click on the form
  2. Click on "copy image" Note:
  3. Open a "new" word document
  4. Paste the copied form in the word document
  5. Click on "Print"

Save the new word document and print for current and future lessons.
